The Hamilton County Health Department inspects restaurants twice a year and grades the businesses on a 100-point scale. Scores below 70 require corrective action. Restaurants are reinspected if they fail to meet critical standards.

To file a health-related complaint, call the Department of Environmental Health at 423-209-8110. The scores are original and do not reflect on-the-spot changes, which sometimes lead to upward adjustments.

83 Alex Thai Food & Sushi Bistro

Reasons included lid in sink. Shrimp was not fully cooked to 145 degrees, was 97-119 degrees Fahrenheit. Brown rice prepared previous day was 45 degrees Fahrenheit, must cool down to below 41 within six hours when the temperature falls below 135 degrees Fahrenheit. Reach-in lowboy refrigerator not cold enough to store or cool rice. Lowboy with raw shrimp not cold enough to hold below 41 degrees Fahrenheit. In-use spoon left in room temperature standing water. Backside door handles on lowboys soiled. Area around dishwashing machine not well maintained and had old buildup. Red tag hanging on hood suppression system. Vents in hood had buildup.

Reasons included hand-washing sinks not accessible. Ready-to-eat food contaminated with water that was pooling in the bottom of the lowboy cooler. Two pounds of pork were discarded. Parasite destruction not listed on seafood invoice. Temperature-control-for-safety foods temped at 49 to 50 degrees Fahrenheit in a lowboy cooler. Observed missing date-marking for ready-to-eat foods in the walk-in cooler. Observed thawing fish at room temperature. Observed uncovered food in the freezer with a buildup of frost. Sugar container was missing a lid as well. Observed wet sanitation cloth resting on a cutting board. Water was pooling at the bottom of the cold-holding lowboy.

Reasons included food prep employee was using cellphone in kitchen area upon inspector's arrival. Food prep employee began food prep without washing hands after cellphone use and after sneezing. Chopped boiled eggs were temping 52 degrees Fahrenheit on salad bar. Live insect activity observed. Gasket to reach-in cooler portion of lowboy refrigerator was in poor repair. Multiple reach-in cooler and freezer bottoms were dirty. Fan units were dirty in reach-in coolers closest to kitchen entrance. There was excessive ice buildup on floor of walk-in freezer. Wall in dishroom was dirty.
