Greene County food inspections find deli selling out-of-date foods

During Greene County food inspections this past week, a health department employee found that a Springfield deli was serving bologna and pasta salad that was five days past its use-by date, among other food items.

The deli at Price Cutter No. 34, 1831 W. Kearney St., also had cheddar broccoli pasta, ham, peppered turkey, and rotisserie chicken for sale, despite all the items being at least one day past the use-by date. The food was removed. Other violations from the Feb. 6 inspection included food not being held at the proper temperatures.

According to the USDA, a "use-by" date is the last date recommended for "the use of the product at peak quality."

Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

Here are more of the food inspections from this past week:

7 Brew Coffee, 1455 W. Battlefield Road, Springfield. Feb. 6 routine inspection – Result: Active. No violations cited at time of inspection.

Arby's Roast Beef Restaurant No. 7725, 1833 W. Republic Road, Springfield. Feb. 6 routine inspection – Result: Permitted with re-inspection. One priority violation: Observed prep cooler next to wall holding sliced tomato and meats at 47 degrees to 51 degrees Fahrenheit. Several items just out of walk-in cooler were at 41 degrees to 42 degrees. Items placed on time control for today; repair company contacted immediately. Required: Cold hold potentially hazardous foods below 41 degrees. One non-priority violation: Observed cutting boards on prep coolers are scored. Required: Replace or resurface so surface is smooth and easily cleanable.

Archie's Italian Eater, 1410 E. Republic Road, Springfield. Feb. 7 re-inspection – Result: active. At time of re-inspection, establishment had replaced old refrigeration unit with new refrigeration unit. No violations noted at time of inspection.

B4 Wings LP, 303 E. Battlefield Road, Springfield. Feb. 9 routine inspection – Result: active. One priority violation: Observed freshly fried chicken temped at 113 degrees. Required: All raw poultry food items shall be cooked to 165 degrees for a minimum of 15 seconds. Corrective action: education provided to employee on correct times to use thermometer to test cooked chicken. Corrected on site. No non-priority violations found.

Bellacino's Pizza & Grinders, 2856 S. Glenstone Ave., Springfield. Feb. 2 routine inspection – Result: active. Observed no hot water at men/women restrooms hand sinks. No priority violations found. One non-priority violation found.

Blue Heron, 206 E. Commercial St., Springfield. Feb. 8 routine inspection – Result: active. No priority violations. Two non-priority violations: Observed door to employee restroom not self-closing, education given; observed vents in walk-in cooler found not clean, education given.

Braum's Ice Cream No. 273, 1401 W. Sunshine St., Springfield. Feb. 8 routine inspection – Result: permitted with re-inspection. Two priority violations: 1. Observed employee remove glove, wipe face on lower forearm and then don new glove. Required: All employees shall keep hands and exposed portions of arms clean. Corrective action: Employee coached when to wash hands and proper areas to wipe face if necessary. Corrected on-site. 2. Observed open front retail cheese cooler air tested at 45 degrees. Boiled eggs tested at 45 degrees internally. Cookie dough tested at 43 degrees. Person in charge stated that coolers go into defrost at 8 a.m. and all temps are taken around 8:15 a.m. Person in charge presented temperature logs at request. Temps recorded in log are done by reading hanging thermometer. No food or air temperatures taken for log records. Required: All food items being cold held shall be held at 41 degrees or below. Corrective action: Hard boiled eggs voluntarily discarded. Discussed with person in charge when recording temperatures to take temperatures of food items. Violation is a repeat offense. Risk control plan will be put in place. Re-inspection is required. Further violations with cold holding upon re-inspection will result in closure.

Bubba's BBQ, 504 N. West Bypass, Springfield. Feb. 8 routine inspection – Result: permitted with re-inspection. A re-inspection will be made on or after Feb. 15 to check for compliance. Four priority violations: 1. Observed staff using lemon-scented bleach to wash dishes, inspector sent staff to get unscented bleach 2. Observed the drain under three-vat sink leaks and bus tub is sitting under it. Observed hot water is shut off at lavatory in men's bathroom due to a leak. 3. Observed evidence of mice in kitchen and water heater room. Areas must be cleaned up and actions taken to minimize mice. 4. Observed cooked meats that had been reheated to 125 degrees, then placed into a hot-hold cabinet that had just been turned on. Meats were removed and heated to 165 degrees. Unit was 148 degrees at 11:15 a.m. Six non-priority violations: 1. Observed floors in the back room, kitchen and walk-in refrigerator were not clean. 2. Observed several food containers not stored inverted. 3. Observed facility was using time as a control on some pre-cooked items but nothing was documented. 4. Observed men's restroom out of paper towels and the paper towel dispenser in women's restroom is not operational. 5. Observed the front counter and some counters in the kitchen were not clean. 6. Observed one chub of raw ground beef was being stored above cooked meat items. This was promptly relocated to bottom shelf at inspector's request.

Casey's General Store No. 3011, 2805 N. Kansas Expressway, Springfield. Feb. 7 routine inspection – Result: active. No priority violations. One non-priority violation: Observed some shelving in walk-in refrigerator and under soda storage was not clean.

Cellar & Plate, 2916 S. Lone Pine Ave., Springfield. Feb. 2 routine inspection – Result: Permitted with re-inspection. Two priority violations: Observed no cold water at kitchen hand sink; observed food debris on can opener. Two non-priority violations: Observed no hand sink in bar; observed no test kit for sanitizer (expired in 2020).

Chinese Chef, 3029 S. Campbell Ave., Springfield. Feb. 6 routine inspection – Result: active. No priority violations. Two non-priority violations: 1. Observed build-up of grease on bottoms of vent hood filters and frame below filters. Clean more often to prevent build-up. 2. Observed fried chicken cooling in walk-in cooler in containers. Chicken is stacked too deep. Required: Place chicken in thin layers so maximum heat transfer can occur. Chicken that is stacked too high does not allow heat to escape, which prevents the food from cooling properly. Chicken was separated into shallow pans, education provided. Corrected on-site.

Conoco Phillips, 2540 N. Glenstone Ave., Springfield. Feb. 3 routine inspection – Result: active. No priority violations. One non-priority violation: Observed mop in sink closet, back wall near soft drink machine in poor repair.

Convenience Plus, 7043 W. State Highway EE, Springfield. Feb. 6 routine inspection – Result: active. Roller grill went out during inspection. No priority violations. Two non-priority violations: Observed soap dispenser in men's restroom was empty; observed cover is missing under mop sink and trim in front of walk-in refrigerator is coming loose.

Crossroads Eagle Stop, 2785 W. State Highway O, Springfield. Feb. 7 routine inspection – Result: active. One priority violation: Observed cleaners stored above single-use coffee lids and cups in storage area. Two non-priority violations: Observed trash cans in women's restrooms didn't have a lid; observed food product stored on the floor of walk-in freezer.

Dennis' Place, 921 W. Sunshine St., Springfield. Feb. 3 routine inspection – Result: active. One priority violation: Observed two separate chemical spray bottles containing chemicals were unlabeled. Required: All working containers filled with toxic materials shall be clearly labeled. Corrective action: Education given and corrected on site. One non-priority violation: Observed men's bathroom floor contained moderate amount of grime build-up. Floor in employee prep area contained moderate amount of grime build-up. Coolers in bar contained grime build-up. Required: Equipment, food-contact surfaces, nonfood-contact surfaces and utensils shall be kept free of an accumulation of dust, dirt, food residue and other debris. Clean equipment and maintain cleanliness.

Dunkin' Donuts/Baskin Robbins, 756 E. US Highway 60 East, Republic. Feb. 6 routine inspection – Result: active. No priority violations. One non-priority violation: Observed no hot water at back hand sink.

Edible Arrangements, 1336 E. Battlefield Road, Springfield. Feb. 8 routine inspection – Result: active. Discussed proper sanitizing procedures and sanitizer testing procedures. No priority violations. Two non-priority violations: Observed no paper towels in dispenser at hand sink in food prep area; observed test strips for quat sanitizer expired.

Fast-N-Friendly, 1451 N. Kansas Expressway, Springfield. Feb. 7 routine inspection – Result: permitted with re-inspection. Walk-in refrigerator was running 46 to 47 degrees and numerous potentially hazardous food items were voluntarily discarded. Three priority violations: 1. Observed walk-in refrigerator running 46 to 47 degrees and numerous potentially hazardous food items were voluntarily discarded. 2. Observed chicken and breakfast sandwiches were 115 degrees and 125 degrees, respectively, in hot hold unit. These were promptly discarded by staff an unit temperature adjusted. 3. Observed open packages of ready-to-eat sausage and hamburger patties in refrigerator that lacked "use by" dates. One non-priority violation: Observed ice scoop being stored in a bucket with about one inch of stagnant water.

Fire & Ice Restaurant - Oasis Convention Center, 2546 N. Glenstone Ave., Springfield. Feb. 6 routine inspection – Result: active. No priority violations. One non-priority violation: Observed ceiling tiles missing outside the dishwashing area and upstairs dishwashing area. Required to keep physical facilities in good repair.

Jimmy's Egg, 3837 S. Campbell Ave., Springfield. Feb. 8 routine inspection – Result: active. One priority violation found: Observed two containers of raw, pooled eggs stored on sheet pan of raw bacon and over ready-to-eat food such as cheese. Required: Separate ready-to-eat food from raw animal foods, store raw animal foods below ready-to-eat foods and in correct order. Corrected on site. Two non-priority violations: 1. Observed build-up of grease on sides of fryer and grill, including wheels of equipment. Required: Clean as often as needed to prevent build-up. 2. Observed floor in the cook line area has grout in poor repair; food items are getting stuck. The floor is hard to clean. Required: The floor should be smooth and easily cleanable. Manager relayed that the floor is scheduled to be re-grouted when the front of house is remodeled. Correct by October 2023.

JRC - North Cafe, 3225 N. Farm Road 123, Springfield. Feb. 8 routine inspection – Result: active. Pizza had been baked within the last hour, so allowed to reheat and put back into hot hold unit. One priority violation: Observed pizza in hot hold unit was 112 degrees, made in last hour. Staff put back into oven and reheated to 165 degrees. One non-priority violation: Observed the thermometer on front of hot hold unit had digital display that was unreadable.

Jyms Investments, LLC, 1550 N. Glenstone Ave., Springfield. Feb. 3 routine inspection – Result: active. No violations cited during inspection.

Kai, 306 S. Campbell Ave., Springfield. Feb. 7 re-inspection – Result: active. On site for re-inspection of faulty cooling units. Upon testing all previously faulty units were recorded to be holding air temperature and food item temperature at 41 F or below. No violations cited at time of inspection.

Kum & Go No. 1463, 3434 W. Chestnut Expressway, Springfield. Feb. 8 routine inspection – Result: active. No priority violations. Three non-priority violations: Observed pizza slice on time only control but not marked with discard time, corrected at time of inspection; observed hot water below 100 degrees in all bathroom hand sinks; observed right-side sink in women's restroom does not stay on for at least 15 seconds.

Kum & Go No. 1470, 1810 E. Kearney St., Springfield. Feb. 9 re-inspection – Result: active. Previous violation items were corrected. No violations cited at time of inspection.

Los Michoacanos Garcias, 1402 W. Sunshine St., Springfield. Feb. 2 routine inspection – Result: permitted with re-inspection. Three priority violations: 1. Observed soup on stove top tested at 124 degrees. Food item was voluntarily discarded. Required: All food items being hot held shall be held at 135 degrees. Corrective action: Education provided that hot held items on stove top must have an active flame heating them. 2. Observed red sauce (contains tomato juice) in two-door Coca Cola machine behind bar tested at 49 degrees. Cream cheese in top section of Berg cooling unit tested 48 degrees. Yogurt in bottom section of Berg cooling unit tested at 46 degrees. Required: All food items behind cold held shall be held at a temperature of 41 degrees or below. Corrective action: Red sauce was voluntarily discarded and education given regarding prepping in smaller batches. Cream cheese from top of berg unit and yogurt from bottom of berg unit voluntarily discarded. Education given on keeping dials on coolers at lower levels to ensure cold holding units maintain potentially hazardous foods at 41 degrees or below. Follow up temperature taken near end of inspection after adjustments were made showed the temperature still at 56 degrees. Re-inspection will occur. 3. Observed multiple items not date marked in walk-in cooler including shredded chicken, beef, queso, etc. Required: All prepared potentially hazardous foods kept over 24 hours shall be date marked not to exceed 7 days. No non-priority violations.

Lucy's Chinese Food II, 2456 E. Sunshine St., Springfield. Feb. 7 re-inspection – Result: active. No violations observed at time of inspection.

Mama Jean's Natural Market, 228 W. Sunshine St., Springfield. Feb. 3 routine inspection – Result: active. One priority violation: Observed cut leafy greens inside walk-in cooler with no date marking. Prepped quinoa placed in sliding door cooler with no date marking. Required: All food items held for over 24 hours shall be date marked for no more than 7 days. Corrective action: Education provided when date marking should take place. Corrected on site. Two non-priority violations: 1. Observed cans of lighter fluid stored above single-serve items. Required: All toxic materials shall be stored so they cannot contaminate food. Corrective action: Education provided where toxic materials can be properly stored, issue corrected on site. 2. Observed multiple dented cans on can storage rack. Required: All foods are to be safe, unadulterated and honestly presented. Corrective action: Education given on when to discard damaged cans. Corrected on site.

McDonald's, 1717 W. Battlefield Road, Springfield. Feb. 7 routine inspection – Result: active. One priority violation: Observed employee handled plastic packaging containing raw hamburger and then proceeded to handle ready-to-eat food packaging without washing hands in between. Required: Employee must wash their hands when switching from raw foods to ready-to-eat foods. Corrective action: Employee coached on when to properly wash hands. Discarded contaminated ready-to-eat packaging and employee washed hands. No non-priority violations.

McDonald's, 2115 E. Independence St., Springfield. Feb. 6 complaint inspection – Result: active. No violations cited at time of inspection.

Nearly Famous Deli and Pasta House, 2708 S. Glenstone Ave., Springfield. Feb. 7 routine inspection – Result: active. No violations noted at time of inspection.

Price Cutter No. 17 Starbucks, 1013 E. US Highway 60 East, Republic. Feb. 6 routine inspection – Result: active. One priority violation: Observed sanitizer below 200 parts per million. Corrected at time of inspection. No non-priority violations.

Price Cutter No. 34 Deli, 1831 W. Kearney St., Springfield. Feb. 6 routine inspection – Result: permitted with re-inspection. A re-inspection will be made on or after 2/9/23 to check priority violations. Five priority violations: 1. Observed the following items on steam table were 115 to 125 degrees: gravy, corn and green beans. These were promptly reheated at inspector's request to 165 degrees. 2. Observed plastic bucket full of wiping cloths being stored in front hand sink. 3. Observed the following potentially hazardous foods were beyond "use by" dates: cheddar broccoli pasta (Feb. 3), Italian pasta salad (Feb. 1), ham (Feb. 3), peppered turkey (Feb. 3), rotisserie chicken (Feb. 5) and bologna (Feb 1). These were promptly removed from customer service. 4. Observed quaternary ammonia solution at three-vat sink was 55 degrees. This was promptly raised to 75 degrees by adding hot water and sanitizer. 5. Observed cooked barbecue ribs and rotisserie chickens held at 44 degrees to 46 degrees in a refrigerated display case. Three non-priority violations: 1. Observed both hand sinks lacked hand soap for employees. Corrected by employee picking up two hand-pump soap dispensers from store and placing by hand sinks. 2. Observed both hand sinks in food preparation area lacked paper towels. Employee went into store and brought back paper towels for both hand sinks. 3. Observed soap dispenser at both hand sinks in food preparation area were non-working.

Price Cutter Plus No. 50 Chop Shop, 3206 E. Battlefield Road, Springfield. Feb. 7 routine inspection – Result: permitted with re-inspection. One priority violation: Observed utensils not stored clean, education given and items were taken to dish area. Two non-priority violations: Observed mop left stored in mop bucket still containing water, education given; observed tea cartons stored in bag on floor, education given and items moved to shelves.

One non-priority violation: Observed hot water in hand-wash station of meat department (shared with sushi) does not turn off completely. Store is turning off water under sink as control. Education given. Leak to be repaired on or before Feb. 21.

Primas East, 3662 S. Glenstone Outer Road, Springfield. Feb. 6 re-inspection – Result: active. At the time of re-inspection, all violations had been corrected.

Prime Inc. Cafeteria (Northstar Cafe), 2740 N. Mayfair Ave., Springfield. Feb. 6 routine inspection – Result: active. One priority violation: Observed two dented cans on shelves in dry storage room. Education given and corrected on-site by staff discarding dented cans. One non-priority violation: Observed food residue on cook line equipment and piping behind cooking line. Required to keep non-food contact surfaces clean from any form of residue or build-up.

S&L Donuts, 280 E. US Highway 60 East, Republic. Feb. 6. routine inspection – Result: active. One priority violation: Observed foods prepared on-site not marked with discard date. Corrected at time of inspection. One non-priority violation: Observed no test strips for chlorine sanitizer used at three-compartment sink.

Storming Crab, 2825 S. Glenstone Ave., Springfield. Feb. 8 routine inspection – Result: permitted with re-inspection. Two priority violations: 1. Observed knives and can opener found not clean. Education given and corrected on site. 2. Observed coleslaw found not at or below 41 (coleslaw 50 degrees). Education given and items voluntarily discarded. Two non-priority violations: Observed interior of rice pot and microwave not found clean, education given; observed hand sink in kitchen had sanitizer but no hand soap, education given.

Subway, 2734 E. Chestnut Expressway, Springfield. Feb. 8 routine inspection – Result: active. Discussed not using expired test strips. One priority violation: Observed insufficient sanitizer in wiping cloth bucket. Corrected at time of inspection. No non-priority violations.
